That weird balloon at the top of the page is coming from the character that’s climbing through the window. You cannot see him climbing through the window because this two-page spread was printed incorrectly – with the left page printed on the right and the right page printed on the next page. It’s a pretty basic formatting thing, but they managed to screw it up anyway.
