At this point, Druuna has been placed under sedation and sent into a virtual world. Presumably she has some kind of control over what she looks like in that world and these are the clothes she chooses to wear.

This page is very telling. She’s talking about how freaked out she is by the mutants around them and the old man laughs and tells her it’s okay – they just want to have sex with her. And he’s tempted to let them do it, for laughs.

It’s a very creepy series. Beautifully drawn, but not so beautifully written.
