These publisher advertisements are always interesting. This one features a lot of work by Giger, who used to show up a lot in earlier issues of Heavy Metal. As with all of these ads, the customer sends the money to Heavy Metal, who presumably takes a cut and sends the order to the publisher for fulfillment (it’s not entirely clear).
